Over 60% of those living with HIV/AIDS are women. The majority of them are in their sexual and reproductive years (UNAIDS, 2006). With antiretroviral (ARV) drugs, most of them are living longer and healthier lives to engage in sexual and reproductive activities (WHO, 2006). This study explored the sexual and reproductive decisions and experiences of women living with HIV/AIDS (WLWHA) in Abuja, Nigeria. Women living with HIV/AIDS in Abuja, Nigeria had unmet sexual and reproductive health needs. Their sexual and reproductive decisions were influenced by their individual circumstances, including their levels of education, poverty, cultural and family influences, partners' HIV status, stigma and discrimination surrounding HIV/AIDS and access to ARV programs. To meet the sexual and reproductive health needs of WLWHA, program planners and policy makers should take these factors into consideration and ensure that programs are comprehensive and integrated.

Despite the growing number of women living with and affected by HIV, there is still insufficient attention to their pregnancy-related needs, rights, decisions and desires in research, policy and programs. We carried out a review of the literature to ascertain the current state of knowledge and highlight areas requiring further attention. We found that contraceptive options for pregnancy prevention by HIV-positive women are insufficient: condoms are not always available or acceptable, and other options are limited by affordability, availability or efficacy. Further, coerced sterilization of women living with HIV is widely reported. Information gaps persist in relation to effectiveness, safety and best practices regarding assisted reproductive technologies. Attention to neonatal outcomes generally outweighs attention to the health of women before, during and after pregnancy. Access to safe abortion and post-abortion care services, which are critical to women's ability to fulfill their sexual and reproductive rights, are often curtailed. There is inadequate attention to HIV-positive sex workers, injecting drug users and adolescents. The many challenges that women living with HIV encounter in their interactions with sexual and reproductive health services shape their pregnancy decisions. It is critical that HIV-positive women be more involved in the design and implementation of research, policies and programs related to their pregnancy-related needs and rights.

The ability of women to safely and effectively control their fertility is a critical health and human rights issue. Family planning provides a range of health, social, and economic benefits for women and their families, yet more than 200 million women worldwide who wish to delay or limit their childbearing are not using any method of contraception. The global health community has recently seen renewed interest in promoting family planning use worldwide. This increased focus on family planning raises questions about how best to deliver reproductive health services to the women who need them, which requires a clearer understanding of how women make decisions about their fertility and how such decisions are carried out. This dissertation explores the reasons why women's stated fertility intentions do not always align with their fertility behaviors or outcomes. The first paper explores the concept of unmet need for family planning by using both quantitative and qualitative methods to better understand why women who want to prevent a pregnancy choose not to use contraception in Luanda Province, Angola. The second paper considers the social context in which women make decisions about their pregnancies in a post-abortion care context in Zanzibar, Tanzania. The third paper examines how women's experiences in early childhood shape their risk of unintended pregnancy later in life in the United States. These dissertation papers provide three distinct examples of how researchers' and policy makers' perceptions of women's reproductive decision-making may fail to recognize or address crucial determinants of fertility intentions, behaviors, and outcomes. In particular, they highlight the importance of considering women's fertility decisions within the context of their full reproductive lives, and point to the need for further research to better understand how such decisions shift over the course of women's lives. Policies and programs that can accommodate how these decisions are formed and changed over time are necessary for ensuring that all women have the right to make decisions about their reproduction and to enact these decisions in safe and effective ways.

In many areas of the globe most HIV infection is transmitted sexually or in association with pregnancy, childbirth and breastfeeding, raising the need for sexual and reproductive health and HIV/AIDS initiatives to be mutually reinforcing. Many people with HIV, who are in good health, will want to have children, and highly active antiretroviral therapy provides women and men living with AIDS the possibility of envisaging new life projects such as parenthood, because of a return to health. However, there are still difficult choices to face concerning sexuality, parenthood desires and family life. Structural, social and cultural issues, as well as the lack of programmatic support, hinder the fulfilment of the right to quality sexual and reproductive health care and support for having a family. This paper addresses the continuum of care involved in parenthood for people living with HIV, from pregnancy to infant and child care, and provides evidence-based examples of policies and programmes that integrate sexual and reproductive health interventions with HIV/AIDS care in order to support parenthood. Focusing on parenthood for people living with and affected by HIV, that is, focusing on the couple rather than the woman as the unit of care, the individual or the set of adults who are responsible for raising children, would be an innovative programmatic advance. Going beyond maternal and child health care to providing care and support for parents and others who are responsible for raising children is especially relevant for those living with HIV infection.
