Profiles of sleep disturbance and sleep quality of children with the specific neurodevelopmental disorders of Smith-Magenis syndrome (SMS), Angelman syndrome (AS), autism spectrum disorder (ASD) and tuberous sclerosis complex (TSC) and the relationships between behavioural and health characteristics, age and sleep were described in these groups. Interview data demonstrated that children with AS's sleep disturbance had a negative impact on both parents and children. A homogeneous sleep disturbance profile of severe night waking and early morning waking affected over 70% of children with SMS but more heterogeneous profiles were found for children with AS, TSC and ASD using cross-group questionnaire data comparisons and when compared to typically developing (TD) children. A heightened risk of sleep-related breathing disorders was identified for children with AS and SMS. Compared to TD children, children with SMS had significantly earlier morning wake times and children with AS and SMS had significantly earlier bedtimes according to actigraphy and sleep diary data. Increased daytime sleepiness in children with SMS was associated with increased overactivity and impulsivity. This thesis includes the largest samples of actigraphy data for children with SMS and AS to date. The importance of aetiology of intellectual disability in the profiling of sleep disturbance was evidenced. Areas for further assessment and intervention include sleep-related breathing disorders for children with AS and SMS and individualized assessment of circadian rhythm disorders for both groups.

Increasingly more and more children with developmental disabilities survive into adulthood. Pediatricians and other clinicians are called upon to care for an increasing number of children with developmental disabilities in their practice and thus there is a need for a practical guide specifically written for paediatricians and primary care clinicians that addresses major concepts of neurodevelopmental pediatrics. In the United States, the specialty training leading to a conjoint board certification by the American Board of Pediatrics and American Board of Psychiatry and Neurology, requires a total of 6 years of training (2 years of pediatrics, 1 year of neurology, 18 months of child neurology, 18 months of neurodevelopmental disabilities). As of December 2006, in the US, there were 241 pediatricians and 55 child neurologists certified in the subspecialty of Neurodevelopmental Disabilities. Thus most of the children with developmental disabilities are seen by pediatricians and therefore it is important for these pediatricians to be well informed of common issues in the field. The 60,000 or so pediatricians in the United States (and hundreds more in other countries) are the main target audience for a practical book on neurodevelopmental pediatrics.

Neurodevelopmental disorders (NDDs) emerge early in life and are associated with functional impairment for children and their families. Amongst children with NDDs, sleep problems are highly prevalent and can have widespread negative effects on children's health, functioning, and quality of life. However, there are few established evidence-based sleep interventions and access to these is limited. This three-study dissertation aimed to lay the groundwork for developing a transdiagnostic eHealth parent-implemented intervention for children with four NDDs (Autism Spectrum Disorder [ASD], Attention-Deficit / Hyperactivity Disorder [ADHD], Cerebral Palsy [CP], and Fetal Alcohol Spectrum Disorder [FASD]). Study 1 is a systematic review of online parent-implemented interventions for NDD symptoms and other behaviour problems in children with NDDs. Twelve interventions were identified, for ASD (n = 8), ADHD (n = 3), and FASD (n = 1). Despite promising evidence for the effectiveness of online parent-implemented interventions and their potential to resolve treatment access problems, no interventions currently qualify as evidence-based and more large-scale trials are required. Study 2 qualitatively explored barriers and facilitators to sleep treatment access, implementation and provision experienced by parents and health care professionals (HCPs) of children with ASD, ADHD, CP and FASD via focus groups / interviews. Similar themes emerged across all NDDs. Key barriers reported by parents (n = 43) and HCPs (n = 44) included lack of knowledge / awareness of sleep problems and their treatment in NDDs, limited access to treatment, the demanding nature of treatments, and parent factors (e.g., exhaustion). Key facilitators included education, support, behavioural treatment approaches, and the ability to modify treatments to account for NDD symptoms. In Study 3, parents of children with ASD, ADHD, CP, or FASD (n = 20) implemented an eHealth parent-implemented insomnia intervention designed for typically developing children, and evaluated the intervention's usability, accessibility, and feasibility. Parents found it usable, acceptable, and feasible, and suggested modifications to make the intervention more useful for children with NDDs. Overall, these studies demonstrate the need for an accessible sleep intervention, and support a transdiagnostic approach to treating sleep problems in children with NDDs.
