Simulation in Acute Neurology is a reference on the execution of a simulation-based educational program in the management of acute neurologic emergencies. Simulation in Acute Neurology has practical value because it contains detailed descriptions of our simulation scenarios. The foundation of this book is our experience with neurosimulation―and it has been a very good one Part I provides an overview of the principles of simulation in medicine and examines the many unique opportunities simulation provides as an educational tool. Barriers to simulating neurologic emergencies are also discussed. Simulation allows a physician-in-training to be observed directly as he or she evaluates and manages acute neurologic disease. Part II is the core of the book. Fifteen acute neurologic emergencies, including complex neuroethical quandaries, are presented in detail, step by step, decision by decision, error after error. Each chapter in this section starts with an explanation of the essence of the discussed neuroemergency (THE PROBLEM BEFORE US), followed by a description of the scenario itself (THE PRESENTING CLINICAL PROBLEM), how scenarios can be adjusted to different types of learners (ADAPTING THE SCENARIO), and ends with a discussion of topics for feedback, which are generally focused around errors and pitfalls (DEBRIEFING). To show the flow of scenarios, we created two additional main headings: (THE IDEAL LEARNER) and (THE NOT-SO IDEAL LEARNER).

This article analyzes the current available literature on high-fidelity simulation as an educational tool in teaching acute neurology and neurocritical care skills to trainees. Twenty-five studies were found to be published in peer-reviewed journals. While every study had a common target of educating learners on appropriate acute neurology evaluation, diagnosis, and management, there exists significant heterogeneity in the chosen topics, implemented procedures, and assessments of learner outcomes. Overall findings suggest that there is considerably significant evidence for the effectiveness and utility of simulation-based education when it comes to the teaching of acute and critical neurological emergencies to trainees and even practicing providers. Existing literature is summarized, current trends are discussed, and recommendations for areas of future research are included

Objective:Medical education innovation with stroke code simulation sustainably prepares neurology residents for participation in the actual stroke code process. Background: Didactic lectures on acute stroke do not prepare neurology residents for nuances of leading a stroke code in high stress environments, multidisciplinary team member interactions, and correct order entry. Simulation technology is increasingly available for resident education, but underutilized in neurology. Design/Methods: A goal-based stroke code curriculum was implemented through 4 scenarios enacted by a standardized patient, nurse, CT technician, stroke fellow, and junior neurology resident at the institution's simulation center. Residents alternated leading or observing simulated codes, followed by group debriefing sessions to review learning goals. Immediate effect of simulation was measured with a pre- and post- multiple-choice quiz and thematic coding of open-ended resident written feedback. Long-term impact of simulation was surveyed with a 5-point Likert scale at 1 year and free text comment box. Results: Between July 2016 and July 2017, 22 junior adult and pediatric neurology residents completed stroke code simulation. Quiz grades improved from 67.5% to 78.3% after simulation. Common feedback themes reference comfort with code logistics, imaging order entry, team member roles, benefit of realistic scenarios, and desire for more scenarios. The residents perceived simulation would relieve the stress of stroke call. At 1 year, all residents continued to feel comfortable with codes including tPA criteria and orders and NIHSS exam. 100% agreed simulation had sufficiently prepared them for stroke call. 83% agreed scenarios reflected their real-life experience. One resident commented stroke simulation was better than any didactic teaching for call preparedness.Conclusions:Medical education innovation with stroke code simulation improves junior neurology resident comprehension of the acute stroke code process and has sustained benefit on preparedness for stroke call at 1 year. Simulation is a valuable tool for neurology resident education.
