Pregnancy stresses the heart and circulatory system. During pregnancy, blood volume increases by 30 to 50 percent. The amount of blood the heart pumps each minute also increases by 30 to 50 percent, and heart rate increases as well. These changes cause the heart to work harder, as do labor and delivery. This issue covers diagnosis and treatment of cardiac symptoms and cardiac emergencies during pregnancy.

The impact of cardiovascular disease on an infant extends from the fetal period to well beyond childhood. Perinatalogists and neonatologists can impact maternal and fetal health through wide range of diagnostic modalities and interventional techniques. For our edition focused on cardiovascular health, we sought to encompass the breadth of knowledge that would be the most relevant for the bed side clinician. Our goal was to assemble contents that would allow a clinician to quickly peruse the journal, and then be prepared to make a medical decision. The interaction between cardiology and perinatology/neonatology includes genetics, diagnostics, interventions, counseling, routine stabilization and day to day care. Ultimately, the goal is to establish the foundation for a healthy adult. For this reason, we have even included chapters on topics that are significant on a day to day basis (such as the proper environment for a newborn) and a long term basis (like the overall neurodevelopmental impact of our interventions). Hopefully, whether in the middle of the night as an emergency reference or during the day as a reliable guide, this edition of Clinics in perinatology will be an important bedside tool for anyone that participates in the care of a patient with perinatal heart disease.

Heart disease is the most common medical cause of maternal death in the UK and it is a major pregnancy problem around the world. The RCOG invited 26 of the world's leading experts from the UK, Europe, the USA and Canada and Japan, to a study group in February 2006, each contributing the latest available information in this vital area. The editors have paid great attention to accuracy but also to readability. The fact that the contributors came together to check, refine and develop their chapters and the speed with which the book has been produced have resulted in a volume which is particularly valuable for updating clinical practice in this key area. This is a book that all obstetricians, cardiologists and anaesthetists dealing with pregnant women with heart disease should read. Anyone with an interest in women's health care will find something for them in these pages.

Cardiac disease is one of the leading causes of maternal morbidity and mortality. Catastrophic outcomes typically encountered are due to gaps in knowledge and communication between health care providers. There is a great need for a standardized approach for care of this very high-risk group of pregnant women. The book encompasses detailed obstetrics and cardiology perspectives that are crucial in the management of the commonly encountered cardiac conditions in pregnancy. This text aims to provide guidance to the whole team caring for a pregnant cardiac patient consisting of obstetricians, maternal-fetal medicine, hospitalists, cardiologists, obstetric anaesthesiologists, emergency physicians, primary care providers and nurses. Features: Cardio-Obstetric team organization Preconception counselling and family planning considerations Cardiovascular disease screening and risk stratification of a pregnant cardiac patient Management of a wide spectrum of cardiovascular diagnoses through use of checklists and algorithms in a simple format Essential key points for each cardiac diagnosis

This is the first textbook to focus on Aortopathy, a new clinical concept for a form of vasculopathy. The first section of the book starts from discussing general concept and history of Aortopathy, and then deals with its pathophysiology, manifestation, intrinsic factor, clinical implication, management and prevention. The second part closely looks at various disorders of the Aortopathy such as bicuspid aortic valve and coarctation of aorta. The book editors have published a lot of works on the topic and have been collecting relating data in the field of congenital heart disease for the past 20 years, thus present the book with confidence. The topic - an association of aortic pathophysiological abnormality, aortic dilation and aorto-left ventricular interaction - is getting more and more attention among cardiovascular physicians. This is the first book to refer for cardiologists, pediatric cardiologists, surgeons, ACHD specialists, etc. to acquire thorough knowledge on Aortopathy.

Simulation is becoming an integral part of medical education and has already a well-established role within anaesthesia training, with many examination questions already modelled on simulation scenarios. The clinical diversity and multidisciplinary nature of an obstetric team creates the ideal environment for simulation training. It enables the team to develop their knowledge of clinical emergencies whilst becoming more aware of the importance of human factors. This book is a practical guide to inspire clinical leads to establish simulation within their delivery suite. The book comprises an extensive obstetric scenario library, with each scenario broken down into learning outcomes, a list of staff and equipment needed, specific stages mapped to learning outcomes with appropriate results/investigations and a checklist of ideal actions to enable the post-scenario debrief. Written by consultant obstetric anaesthetists, this book will appeal to medical practitioners interested in facilitating obstetric simulation.
