Events in a neurological intensive care unit are not always predictable and patients can often be unstable. This practical manual is a clear and concise guide for recognising and managing neurological emergencies. Each chapter covers a crucial topic in neurocritical care, from understanding the pathophysiology of various neurological diseases, to neuroradiology used in diagnosis, and best practice for difficult decision making in the ICU. A variety of conditions are described such as haemorrhage (intracerebral, subdural, and subarachnoid), seizures, trauma and temperature dysregulation. An international team of experts have contributed chapters, providing a breadth of experience and knowledge for readers. This is an invaluable guide for clinicians on the front line of caring for patients with neurological emergencies who need life-saving answers quickly.

This updated and refined new edition is the only book to provide a comprehensive approach to the intensive care of neurologically injured patients from the emergency room and ICU through the operating room and post-surgical period. It reviews neuroanatomy, neuroradiology, and neurophysiology, examines the neurological problems most frequently seen in intensive care, and describes the various types of neurosurgery. General issues are discussed, such as cardiac care, fluids and electrolytes, nutrition, and monitoring as well as more specific conditions and complications including elevated intracranial pressure, seizures, and altered mental states.

In recent years, neurocritical care has grown and matured as a subspecialty of Critical Care Medicine with the advent of new monitoring, diagnostic, and therapeutic capabilities. The goal of neurocritical care is to rapidly deliver acute medical therapies and appropriate interventions through effective monitoring of both neurological and end organ function. Neurocritical Care provides 'at the bedside' guidance on the medical knowledge and technical skills required to care for critically ill patients with neurologic conditions such as cerebrovascular disorders, neurotrama, neuro-oncology, refractory seizures, neuromuscular diseases, infections, alterations in consciousness, and perioperative neurosurgical care. Part of the Pittsburgh Critical Care Medicine series, this compact volume is an ideal reference for physicians and trainees working in either a general ICU or specialty Neuro ICU unit. Readers will gain an understanding of background knowledge and concrete solutions to questions and situations encountered in daily practice.

This is a practical and accessible review of neurologic critical care in the intensive care unit is single-authored and thus cohesive. The emphasis is on management in day-to-day practice. For the thoroughly updated and expanded second edition, Wijdicks has added new algorithms on outcome prediction in the specific disorders, and five chapters on the organization of the intensive care unit, acute spinal disorders, management of common postoperative neurosurgical complications, and psychosocial issues, ethics, and withdrawal of life support. For quick reference in the ICU the most useful tables and figures have been extracted and reprinted in an accompanying pocket-sized booklet.

The perioperative care of individuals with neurologic compromise is critically important, yet it is only one dimension of the rich relationship between anaesthesiology and the neurosciences. The mechanism of everyday therapeutic interventions such as anaesthesia and analgesia is exciting neuroscience in its own right. At the new frontier of outcomes studies lies the question of how the perioperative period might impact the brain. For example, questions related to anaesthetic neurotoxicity, delirium, and cognitive dysfunction pose critical challenges for the field. The Oxford Textbook in Neuroscience and Anaesthesiology addresses the exciting field of neuroanaesthesiology in a new and stimulating way. In twenty eight chapters, the neuroscientific basis of anaesthesiology, the full spectrum of clinical neuroanaesthesia, and the care of neurologic patients undergoing non-neurologic surgery are explored in one comprehensive textbook for the first time. The first section considers the neural mechanisms of general anaesthetics, cerebral physiology, the neurobiology of pain, and more. The second section explores the care of patients with neurologic disease in the operating room or intensive care unit. These clinical chapters systematically treat the perioperative considerations of both brain and spine surgery, and provide introductions to neurocritical care and pediatric neuroanaesthesia. The final section outlines the care of neurologic patients undergoing non-neurologic surgery. It examines key connections of neurology and anaesthesiology, examining how conditions such as dementia, stroke, or epilepsy interface with the perioperative period. Each chapter has been carefully crafted to be concise yet highly informative, reflecting the cutting edge of neuroscience and neuroanaesthesiology. This international textbook gathers the best available expertise of authors and leaders worldwide. Includes access to online-only content, including more than 20 cases and more than 90 questions that can be used in presentations and teaching sessions. Brain injury is a worldwide leading cause of mortality and morbidity and requires early and appropriate management to minimize these adverse sequelae. Despite such needs, access to specialist centers is limited, forcing both immediate and secondary care of these patients onto generalist staff. These responsibilities are made more problematical by differences in patient management between and even within specialist centers, due in part to an insuffcient evidence-base for many interventions directed at brain injury. This book is borne out of the above observations and is targeted at em- gency and acute medicine, anesthetic and general intensive care staff caring for brain injury of diverse etiology, or surgical teams responsible for the inpatient care of minor to moderate head trauma. Although explaining the various facets of specialist care, the book is not intended to compete with texts directed at neurosciences staff, but aims to advise on optimal care in general hospitals, including criteria for transfer, by a combination of narrative on pathophysiology, principles of care, templates for documentation, and highly specifc algorithms for particular problems. It is intended that the content and structure can form the basis of guidelines and protocols that refect the needs of individual units and that can be constantly refned. Our ultimate goal is to promote informed, consistent, auditable, multidisciplinary care for this cohort of patients and we hope that this text contributes to that process.
