Nowadays, peri-implantitis, a destructive inflammatory process around implants contributing to bone loss, has become the main reason for implant failure. Bacterial colonization and subsequent biofilm (plaque) formation on the implant surface is one of the main factors causing peri-implantitis. Various antibacterial coatings have been tested. Totarol is a kind of natural antibacterial agent, with bactericidal effect towards a wide range of gram-positive, even antibiotic-resistant bacteria including the methicillin-resistant S. aureus, and additionally with low cytotoxicityufffc. This in vitro study aims at applying totarol coatings on experimental dental implant and abutment surfaces and investigating the antibacterial capability of totarol coatings in a simulated oral environment. The natural antibacterial totarol coating showed promising bactericidal effects towards the early oral colonizer S. gordonii and the multispecies oral bacterial culture, and simultaneously significantly inhibits their biofilm growth.

This work presents the design and development of a novel multi-functional coating, ionic liquid-based, for surface improvement of dental implants. Dicationic imidazolium-based ionic liquids (ILs) were rationalized, synthesized and their molecular structure and thermal behavior were characterized. IL toxicity against pre-osteoblast (MC3T3-E1) and human gingival fibroblast (HGF-1) cells, as well as the antimicrobial activity against bacterial strains relevant to the oral environment, were evaluated. For some IL compositions it was established a safe range of concentrations that ILs can be used, in which the minimum inhibitory concentration (MIC) against bacteria was lower than the concentration that inhibits 50% of cellular enzymatic activity (IC 50). These compounds were used as coatings of commercially pure titanium (cpTi) samples and the coated and non-coated samples were characterized. X-Ray photoelectron spectroscopy was used to understand the intermolecular interactions between IL molecules and Ti surface. Atomic force microscopy (AFM) provided a morphological map of surfaces as well as adhesion force of coatings and UV-Vis spectroscopy was used to understand their release profile. ILs were also tested in regards to their anti-corrosive and lubricant activities and it was observed that these compounds form stable films on the surface providing corrosion protection and lubrication. Finally, the coated surfaces were observed to exert antimicrobial and anti-adhesive activities against bacterial strains while also preserving the compatibility of the surface with gingival fibroblast and bone-forming cells.

In this chapter, a general overview of current dental implants is presented, identifying weak points where improvements can be made. Particular importance is given to the surface properties of materials susceptible to modifications for improving the biological response for the specific application. Then, the concepts and techniques relevant for the evaluation of the physico-chemical properties and the protein adsorption, biocompatibility, bioactivity and biofilm formation are described.
