There has been a tremendous increase in interest in the neuropathogenicity of viruses during the past decade as we have come to recognize that the human immunodeficiency virus, which causes the acquired immunodeficiency syndrome (AIDS), can infect glial cells and cause neurological disease. Yet this increase has not been limited to AIDS but has extended to viruses that infect either or both the central and peripheral nervous systems. The changes examined here include both neurological and psychological diseases or syndromes. Moreover, the chapters in this volume review the interaction of the host immune system with the viruses examined and how such interactions may increase or decrease the neuropatho genicity of the viruses. Questions regarding viral neuropathogenesis include: (I) What is the mode of transmission of virus to the nervous system? (2) What types of cells are infected, and do they contain receptors for the virus? (3) What is the extent of damage that results from viral infection? (4) What are the immunologic mecha nisms by which damage is mediated or limited? Many of these questions remain unanswered, but this volume delves into efforts to provide some answers.

Infections of cells within the brain can occur following challenge with many viruses and other pathogens. Such infections often result in the activation of parenchymal cells as well as substantial infiltration of blood borne lymphocytes. Recent discoveries reject the notion that the central nervous system (CNS) is “immune privileged”. However, it is also true that immune effectors and outcomes are notably different from canonical responses that occur in the periphery. I have investigated the contribution of T cells and B cells in controlling a neuron restricted virus infection, and have discovered that CD4+ T cells play a critical role in immune cell education and consequences following challenge. Specifically, I have shown that survival of permissive mice challenged with measles virus (MV) is CD4+ T cell dependent, but that either B cells or CD8+ T cells must also be present to ensure pathogenesis-free survival. Interestingly, even with an intact immune response, MV RNA is detectable in healthy immune competent adult mice 90-120 days post infection (dpi), suggesting that sterile clearance of infection does not occur in the CNS, allowing the establishment of a life-long subacute infection. In addition, I sought to determine whether the host response to multiple, concurrent antigenic challenges could interact and disrupt the finely tuned balance that occurs in the single neurotropic MV challenge model. To do so, a two virus, tissue-restricted model was developed in which mice are infected with neurotropic MV and viscerotropic lymphocytic choriomeningitis virus (LCMV). Double-infected mice experience a fatal neuropathogenic disease following co-infection, resulting from edema mediated by CD8+ T cells. To better understand how a concurrent immune response could alter a non-pathogenic MV response in the brain, I used an adoptive transfer approach, and showed that, while CD8+ T cells are necessary for pathogenesis, they are not sufficient. Rather, CD4+ T cells, activated by an allogeneic immune challenge, appear to “convert” MV-specific CD8+ T cells to a neuropathogenic phenotype. Taken together, these results further dissect the complex interplay of T lymphocytes following neurotropic virus infection, and have revealed novel pathogenic mechanisms that occur in the CNS following polymicrobial challenge.

In spite of progress in biomedical research, we know little about the causes, prevention, and treatment of the numerous mental and neurological disorders that afflict up to 15% of all individuals. In the last decade, great advances have been made in the physiopathology of mental and neurological disorders, leading to at least a partial control of Parkinson's disease, epilepsy, certain psychoses, and anxiety syndromes. Despite the fact that an underlying specific neurotransmitter deficiency has been demonstrated in Alzheimer's and Parkinson's diseases, the immune dysfunction and viral hypotheses continue to be attractive for investigators dealing with these degenerative diseases of the aging brain, which afflict 10% of senior citizens. A retrospective epidemiologic study suggests that the encephalitis lethargica and parkinsonism were almost certainly caused by the 1918 influenza virus pandemics. It must be stressed that the etiopathogenesis of many mental disorders is not known, and this ignorance has led to several untenable neurophysiological and biochemical hypotheses. Epidemiologic investigations show a high prevalence of functional psychoses and organic mental disorders. Although many of them are conceptualized as biopsychosocial disorders, recent data indicate that the biological component appears more and more as a major etiologic factor. Among the various biological hypotheses, the viral and im munologic concept has become a significant one. In view of recent discoveries in virology and immunity, it becomes clear that viral and immunologic hypotheses should be inves tigated more systematically concerning the mechanisms of numerous mental and neu rological disorders.

Although viruses have been implicated in central nervous system (CNS) diseases of unknown etiology, such as multiple sclerosis (MS) and amyotrophic lateral sclerosis (ALS), the identification of viral triggers in such diseases has been largely unsuccessful. To determine if peripheral infections could contribute to neuropathogenic disease, we developed a mouse model in which immune cells generated to a peripheral infection are recruited to the CNS, where they then mediate immunopathogenesis. In this model, mice are infected with both CNS-restricted measles virus (MV), and peripherally restricted lymphocytic choriomeningitis virus (LCMV). While infection with either virus alone resulted in no illness, co-infection caused disease in all mice, with approximately 50% of these mice dying from seizures. Coinfection in all mice correlated with a 12-fold increase in the number of CD8+ cells in the CNS as compared to MV infection alone. Tetramer analysis revealed that at least 50% of these infiltrating CD8+ T cells were of LCMV-specificity Challenge of mice with other combinations of peripherally and CNS-restricted viruses resulted in an identical immunological outcome. Thus, despite no apparent viral cross-reactivity, activated lymphocytes specific for a peripheral infection are readily recruited to the CNS, triggering neurological disease that does not occur with either infection alone. In this model, disease appears to be attributable to massive edema and transtentorial brainstem herniation, similar to that observed in mice challenged intracerebrally (IC) with LCMV. These results indicate that T cell misrecruitment under conditions of concomitant immune insults can trigger novel neuropathogenic outcomes.
