Doctoral Thesis / Dissertation from the year 2014 in the subject Medicine - Biomedical Engineering, grade: Excellent "cum laude", , language: English, abstract: Minimally invasive surgery (MIS) has become in many surgical specialties and procedures the gold-standard choice due to its efficiency and benefits towards patient safety. However, the introduction of laparoscopic surgery has led to the need of developing new surgical skills different to those required for open surgery, with a significant learning curve to perform a safe laparoscopic surgery. Traditional subjective assessment methods of trainees are no longer adequate for surgical training. Reduced working hours as well as demands from surgeons and associations mean that more objective assessment tools that can accredit surgeons as technically competent are required. Evidence exists to validate motion analysis for laparoscopic technical skills assessment. Motion analysis of the laparoscopic instruments seeks to determine aspects that indicate the difference between surgeon's level of surgical dexterity. However, at the moment there is not an extended method to be used with current available training systems used in training laboratories as well as for the OR. The purpose of this book is to present the design, development and validation of three novel motion analysis methods focused on the use of real laparoscopic instruments during laparoscopic performance. These methods are based on computer vision techniques attempting to not interfere with the surgical practice. They are introduced in an evolutionary way from methods for exclusive use in a box trainer to solutions with the potential of being used in actual OR setting. Overall, this work corroborates the research hypothesis regarding the use of three different video-based tracking technologies for motion analysis of laparoscopic instruments, the use of instrument motion analysis for MIS technical skills assessment and the relationship between motion-related assessment metrics and quality of technical performance in laparoscopic training. These presented methods provide a tool to objectively assess MIS technical performance and a support to train novice surgeons in MIS techniques. The findings of this work encourage us to continue researching in improving these methods to be introduced as part of an actual laparoscopic training program, both inside and outside the OR.

For many procedures, open surgery is being replaced with minimally invasive surgical (MIS) techniques. The advantages of MIS include reduced operative trauma and fewer complications leading to faster patient recovery, better cosmetic results and shorter hospital stays. As the demand for MIS procedures increases, effective surgical training tools must be developed to improve procedure efficiency and patient safety. Motion tracking of laparoscopic instruments can provide objective skills assessment for novices and experienced users. The most common approaches to noncontact motion capture are optical and electromagnetic (EM) tracking systems, though each approach has operational limitations. Optical trackers are prone to occlusion and the performance of EM trackers degrades in the presence of magnetic and ferromagnetic material. The cost of these systems also limits their availability for surgical training and clinical environments. This thesis describes the development and validation of a novel, noncontact laparoscopic tracking system as an inexpensive alternative to current technology. This system is based on the fusion of inertial, magnetic and distance sensing to generate real-time, 6-DOF pose data. Orientation is estimated using a Kalman-filtered attitude-heading reference system (AHRS) and restricted motion at the trocar provides a datum from which position information can be recovered. The Inertial and Range-Enhanced Surgical (IRES) Tracker was prototyped, then validated using a MIS training box and by comparison to an EM tracking system. Results of IRES tracker testing showed similar performance to an EM tracker with position error as low as 1.25 mm RMS and orientation error

In this thesis, we propose a framework for automated assessment of surgical skills to expedite the manual assessment process and to provide unbiased evaluations with possible dexterity feedback. Evaluation of surgical skills is an important aspect in training of medical students. Current practices rely on manual evaluations from faculty and residents and are time consuming. Proposed solutions in literature involve retrospective evaluations such as watching the offline videos. It requires precious time and attention of expert surgeons and may vary from one surgeon to another. With recent advancements in computer vision and machine learning techniques, the retrospective video evaluation can be best delegated to the computer algorithms. Skill assessment is a challenging task requiring expert domain knowledge that may be difficult to translate into algorithms. To emulate this human observation process, an appropriate data collection mechanism is required to track motion of the surgeon's hand in an unrestricted manner. In addition, it is essential to identify skill defining motion dynamics and skill relevant hand locations. This Ph. D. research aims to address the limitations of manual skill assessment by developing an automated motion analysis framework. Specifically, we propose (1) to design and implement quantitative features to capture fine motion details from surgical video data, (2) to identify and test the efficacy of a core subset of features in classifying the surgical students into different expertise levels, (3) to derive absolute skill scores using regression methods and (4) to perform dexterity analysis using motion data from different hand locations.
