This book is the first in a series of planned volumes focused on preserving the character of the development of bioethics in particular cultural contexts. As the first of these volumes, Leo Pessini, Christian de Paul de Barchifontaine, and Fernando Lolas Stepke’s work has succeeded well. It has brought together accounts by sch- ars who were crucial to the emergence of bioethics in the Ibero-American cultural domain. This trail-blazing work in the history of bioethics will be of enduring s- nificance. I am deeply in their debt for having shouldered this far from easy task. Bioethics is the product of very particular socio-historical developments. Most prominent among them have been (1) the secularization of the dominant culture of North America, Western Europe, and now Central and South America as well, (2) a deflation of the status and authority of physicians as moral authorities able to guide their own profession, and (3) the salience of a post-traditional animus that gives c- tral place to persons as isolated atomic sources of moral authority. Bioethics initially took shape in North America as a post-Christian, post-professional, post-traditional social movement. This bioethics sought to establish a moral discourse for the public forum, a moral practice able to give practical guidance in hospitals and other insti- tions, and a body of undergirding and justifying theoretical reflections.

Bioethics developed as an academic and clinical discipline during the later part of the 20th century due to a variety of factors. Crucial to this development was the increased secularization of American culture as well as the dissolution of medicine as a quasi-guild with its own professional ethics. In the context of this moral vacuum, bioethics came into existence. Its raison d’être was opposition to the alleged paternalism of the medical community and traditional moral frameworks, yet at the same time it set itself up as a source of moral authority with respect to biomedical decision making. Bioethics serves as biopolitics in so far as it attempts to make determinations about how individuals ought to make medical decisions and then attempts to codify that in law. Progressivism and secularism are ultimately the ideology of bioethics.

This book presents and elaborates on how the teaching of global ethics in healthcare contributes to furthering ideals of cosmopolitanism: solidarity, equality, respect for differences and concern with what human beings, and specifically patients have in common, regardless of where they live and who they are. Global problems such as pandemic diseases, disasters, lack of care and medication, homelessness and displacement call for global responses. The new area of global bioethics is providing answers by arguing that ethical discourse should first of all criticize the structures of violence and injustice that underlie many threats to global health. Education of health professionals should articulate that they are ‘citizens of the world’, like their patients. This book first demonstrates that a moral vision of global education is necessary to gain a global dimension. It is argued that a global framework of ethical principles is available; the challenge is to elaborate and specify that framework into specific educational approaches and models. The book subsequently analyzes goals and challenges of global education in biomedicine and healthcare. It is shown how such challenges (e.g. inequities and cultural differences) can be overcome. Finally, the book presents concrete examples (cases, methods, and practices) of global education in bioethics. The unique feature of the book is that it addresses global education challenges specifically in the area of healthcare, medicine, and medical science. It combines two areas of research and experience that are usually not connected: global bioethics and global education. This book discusses the philosophical foundations of bioethics, with a particular focus on the tensions and potential dilemmas generated by the intuitionist meta-ethical commitments of the predominant normative theory, namely “the four principles approach.” This view is based on the prima facie norms of respect for autonomy (one ought to respect the autonomous choices of subjects of scientific research/patients), non-maleficence (one ought to refrain from inflicting harm), beneficence (one ought to do good and prevent, or remove, harm) and justice (one ought to treat people fairly). The tensions in applying these basic principles may lead to inaction in scientific experiments involving human subjects or to arbitrary applications of the norms in the art of caring. The problem can be made explicit in these terms: on the one hand, caring without respecting seems blind, degenerating into forms of paternalism when, for instance, the carer imposes her conception of the good life or a particular procedure on the cared-for; on the other hand, respecting without caring amounts to indifference or individualism when, for example, a person does not look after a vulnerable being properly. The initial hypothesis of this book, then, is that the concept of respectful care can be built up, working from an ethico-philosophical perspective, to be a leading notion capable of guiding our daily actions and bioethical practices.
