"Trying to understand how normal brain function is disrupted in epilepsy, a growing number of studies have focused on the analysis of resting-state fMRI by calculating measures of functional connectivity. However, it is still unclear whether resting-state fMRI can help exploring alterations in brain functions in this disease. Moreover, it has not been investigated whether the methods of functional connectivity analysis are appropriate for detection of these variations. Finally, not enough research has been performed to understand the mechanisms underlying different analysis methods and to find how they should be interpreted. This dissertation looks into these issues in a systematic way and to this end three experiments were performed.The first study, in patients with idiopathic generalized epilepsy, aimed at providing a unified approach to explore changes in resting-state brain function of the regions normally involved in sustained attention. We used seed-based functional connectivity analysis, which allowed us to detect functional connectivity changes related to the duration of epilepsy. We identified three networks in which functional connectivity was related to the duration of epilepsy. Our findings were specific to these networks, as none of the additionally tested seeds resulted in correlation between the duration of epilepsy and functional connectivity.In order to have a network view of the changes all over the brain, we decided to go toward data-driven approaches and considered independent component analysis (ICA). In a study not included in this thesis, we first tried to solve the problems of available ICA-based functional connectivity approaches in conducting between-group comparisons. We proposed a method, called shared and specific ICA, or SSICA, which enabled us to systematically extract and classify components as shared by the groups and specific to a group. In study two, we included a finger-tapping dataset and generated realistic simulations (hybrid data) by adding arbitrary focal activations (patches) to the resting-state data of healthy subjects, and tested the validity of SSICA. In addition to examining the SSICA robustness with respect to several internal parameters and initializations, we provided several simulations to investigate the characteristics of SSICA in different real-case situations. We compared the results of SSICA with a current group-ICA method. Results showed that for both cases and in different situations, SSICA outperforms the current approach in terms of performance, sensitivity and robustness. The third study applied SSICA on data of patients with mesial temporal lobe epilepsy (MTLE) and compared the resting-state brain function between these patients and healthy controls. We applied SSICA multiple times with different internal parameters and initializations and this resulted in a large number of components specific to the patients and to the controls. To identify the most reliable and reproducible specific networks, we applied a spatial clustering analysis. Finally, we estimated the functional connectivity in each group using power spectrum analysis. We found two reliable MTLE-specific networks and one control-specific network. We showed that the two reliable MTLE-specific networks are in fact brain regions with higher functional connectivity in patients compared to controls whereas the regions involved in the control-specific network show the opposite. Overall, our studies indicate that resting-state functional connectivity can be used as a viable approach to detect alterations in resting brain function in epilepsy and that SSICA is a more appropriate method than current group-ICA approach to detect between-group differences. Moreover, our findings demonstrate that the use of SSICA, not only makes a priori hypotheses unnecessary but also permits differences between the groups to be detected with high reliability and sensitivity." --

The goal of this book is to make a link between fundamental research in the field of cognitive neurosciences, which now benefits from a better knowledge of the neural foundations of cerebral processing, and its clinical application, especially in neurosurgery – itself able to provide new insights into brain organization. This research focused on the development of a methodology for analyzing functional magnetic resonance imaging (fMRI) data collected from patients with epilepsy in order to map epileptic networks. Epilepsy, a chronic neurological disorder characterized by recurrent, unprovoked seizures, affects up to 1% of the world's population. Antiepileptic drug therapies either do not successfully control seizures or have unacceptable side effects in over 30% of patients. Approximately one-third of patients whose seizures cannot be controlled by medication are candidates for surgical removal of the affected area of the brain, potentially rendering them seizure free. Accurate localization of the epileptogenic focus, i.e., the area of seizure onset, is critical for the best surgical outcome. The main objective of the research was to develop a set of fMRI data features that could be used to distinguish between normal brain tissue and the epileptic focus. To determine the best combination of features from various domains for mapping the focus, genetic programming and several feature selection methods were employed. These composite features and feature sets were subsequently used to train a classifier capable of discriminating between the two classes of voxels. The classifier was then applied to a separate testing set in order to generate maps showing brain voxels labeled as either normal or epileptogenic based on the best feature or set of features. It should be noted that although this work focuses on the application of fMRI analysis to epilepsy data, similar techniques could be used when studying brain activations due to other sources. In addition to investigating in vivo data collected from temporal lobe epilepsy patients with uncertain epileptic foci, phantom (simulated) data were created and processed to provide quantitative measures of the efficacy of the techniques.
