Over the past decade a great deal of research activity has occurred on either side of the gastroduodenal epithelial interface, with the common goal of elucidating the mechanisms of mucosal protection and how these may be compromised in peptic ulcer disease. A prime stimulus to such research has been the realization that abnormal acid secretion or its delivery into the duodenum in excessive amount cannot fully explain ulcer pathogenesis in the majority of patients. One may envisage the stomach and proximal duodenum as being in a dynamic equilibrium, with aggressive luminal factors, such as acid and pepsin, being counteracted by protective mucosal mechanisms. Until recently, the mechanisms involved in mucosal protection had been poorly defined while the physiology and pathophysiology of acid and pepsin secretion had been elucidated. The impression that gastroduodenal mucosa was protected by a single mechanism has at last been replaced by a more realistic view, suggesting a number of protective zones acting in series. Some of these zones may act as first or second line defences against aggressive factors while certain zones may specifically protect against certain aggressors.

First published in 2001. This book is an invaluable clinical reference for the therapeutic management of gastrointestinal and liver diseases which the gastroenterologist and trainee encounter in their practice. Various drugs and their interactions with their intended site and also with other drugs are discussed from a practical point of view. This book is not a treatise on gastrointestinal pharmacology but a practical guide to the therapeutic use of GI drugs.

The application of molecular techniques to gastroenterology continues to yield important advances in the development of drugs to treat gastrointestinal disorders. Important new drugs have emerged through the collaborative and complementary efforts of basic scientists, clinicians, and clinical researchers in academia and the pharmaceutical industry. The challenge has been exciting, with a few surprises along the way. Consider peptic ulcer disease as an example. The discovery of H receptors and the availability of potent and 2 selective H-receptor antagonists signaled the beginning of a new era 2 in the treatment of gastric hypersecretory states and peptic ulcers. Introduction of proton pump inhibitors offered another therapeutic option. Though H-receptor antagonists and proton pump inhibitors 2 are important and useful drugs, the discovery of the link between H. pylori infection and peptic ulcer disease has led to even more effective pharmacotherapeutic regimens. Our intent in Drug Development: Molecular Targets for GI Diseases is to bring together hands-on experts to review promising areas of gastrointestinal pharmacology. The contemporary topics covered, from a mechanistic viewpoint, are relevant to gastrointestinal inflammation and motility disorders. Authoritative opinions are offered on both future research directions and potential applications for new therapies.

In this book the pathophysiology, diagnosis, and therapy of most aspects of peptic ulcer disease are examined in detail. Covered are the pharmacology of the parietal cell and the latest hypotheses for the development of acid-peptic disease including epidermal growth factor, gastrin and other peptide hormones, fatty acids and prostaglandins, helicobacter pylori, and gastric acid hypersecretion including Zollinger-Ellison syndrome. The book also deals with the newer antisecretory medication such as omeprazole with a review of H2-receptor antagonists. Rounding off the subject are chapters on gastroesophageal reflux disease, upper GI endoscopy, and videoendoscopy including digital imaging. This volume addresses a wide range of physicians who are interested in all aspects of acid-peptic disease. What distinguishes this volume from others is the timely update on a number of important issues pertaining to acid-peptic disease. Not only does it review a number of topics that are of great interest at the present time, but it introduces a vast amount of new unpublished data.
