The main purpose of this report is to assess the degree to which progress has been made by the Government of Uzbekistan in increasing the transparency, accountability, and efficiency of its primary health services, and to identify what further steps could be taken to improve the effectiveness of delivery, some of which may be applicable to other sectors as well. The report examines public spending, public financial management, and human resource management and remuneration in the primary health care sector at the local level. It draws lessons from the experience of Ferghana and Samarkand, two oblasts that have implemented reforms in primary health care, and incorporates the findings drawn from other sources, such as public expenditure data, the laws and regulations governing these systems, and the Service Quality Delivery Survey (SQDS) conducted in 2007. This report discusses the key governance issues related to improving service delivery in the primary health care area, and concludes with recommendations tailored to decision makers at the central and local government levels and those in local facilities.

The Health Systems in Transition (HiT) profiles are country-based reports that provide a detailed description of a health system and of policy initiatives in progress or under development. HiTs examine different approaches to the organization, financing and delivery of health services and the role of the main actors in health systems; describe the institutional framework, process, content and implementation of health and health care policies; and highlight challenges and areas that require more in-depth analysis. The Uzbek health system has undergone significant changes since the country became independent in 1991. While Uzbekistan has made progress in the restructuring of different layers of health services with an increased emphasis on primary care, the coordination of different levels of care remains a major challenge. The establishment of a state-guaranteed benefits package was an important element of health reforms. However, a number of essential services were left outside the state-guaranteed benefits package for the majority of the population, including secondary and tertiary services and outpatient pharmaceuticals. This has created many challenges, such as increasing the pressure on emergency services, which are comparatively well equipped and formally free of charge. Overall, access to secondary and tertiary care seems to have deteriorated in recent years and out-of-pocket payments (both formal and informal) present a major barrier to accessing health services and pharmaceuticals, in particular for low-income groups. In addition, there are significant differences in terms of per capita health expenditure across regions and many rural physician points face staffing shortages. Quality improvement is another major challenge for the Uzbek health system and initial quality improvement initiatives are now being undertaken. [Ed.]

Strategic purchasing of health services involves a continuous search for the best ways to maximize health system performance by deciding which interventions should be purchased, from whom these should be purchased, and how to pay for them. In such an arrangement, the passive cashier is replaced by an intelligent purchaser that can focus scarce resources on existing and emerging priorities rather than continuing entrenched historical spending patterns.Having experimented with different ways of paying providers of health care services, countries increasingly want to know not only what to do when paying providers, but also how to do it, particularly how to design, manage, and implement the transition from current to reformed systems. 'Designing and Implementing Health Care Provider Payment Systems: How-To Manuals' addresses this need. The book has chapters on three of the most effective provider payment systems: primary care per capita (capitation) payment, case-based hospital payment, and hospital global budgets. It also includes a primer on a second policy lever used by purchasers, namely, contracting. This primer can be especially useful with one provider payment method: hospital global budgets. The volume's final chapter provides an outline for designing, launching, and running a health management information system, as well as the necessary infrastructure for strategic purchasing.
