Endophthalmitis is a serious intraocular infection that occurs most commonly due to complications of intraocular surgery and often causes severe visual impairment or even the loss of an eye. The incidence of postoperative endophthalmitis varies by the specific surgical procedure and technique performed, but the overall incidence has been declining since the late 19th to 20th century. The risk of developing postoperative endophthalmitis is associated with a number of factors such as conjunctival diseases, skin diseases, and complications during surgery. The most common bacteria causing postoperative endophthalmitis are presumably derived from patients' conjunctiva and eyelids during surgery. To verify this, The Endophthalmitis Vitrectomy Study evaluated 420 postoperative patients with endophthalmitis presenting within 6 weeks of surgery. This study was to determine the necessary of immediate vitrectomy versus antibiotic therapy based on the type of bacteria detected. Because of the advancements in aseptic techniques and the use of prophylactic antibiotics, a better understanding of the causes of the infection has resulted in a dramatic decrease of infection since the 1960's and 70's.
