In Dec., 2007, during the course of an unrelated investigation being conducted by this office, we discovered a flyer at the Columbus State Community College Web site announcing auditions for an upcoming theatrical production of "A raisin in the sun." One of the contact persons on the flyer for these auditions was James Mathis, an employee at the Bureau of Workers' Compensation ("BWC"). Mathis' state telephone number and e-mail address were listed as contact information for anyone interested in auditioning for the production. We contacted BWC's Human Resources Department and Special Investigations Department for their assistance in retrieving records and other data on Mathis. A preliminary review of these records showed that a large volume of Mathis' e-mail traffic involved non-work related activities. We also found that Mathis had been involved in several musical and theatrical productions and had coordinated portions of these while on state time. We further learned that he had been compensated for his work on these productions.

On Dec. 6, 2007, Ohio Bureau of Workers' Compensation ("BWC") Director of Special Investigations Department Thomas Wersell reported to the Deputy Inspector General for BWC/Industrial Commission that he had been informed of an allegation that BWC External Auditor Ward Snyder may have submitted audits with the forged signatures of employers. Wersell also reported that Snyder may not have been performing on-site audits as required.

On May 5, 2010, the Office of Inspector General initiated an investigation after being notified of an allegation concerning Ohio Bureau of Workers' Compensation Staff Attorney Julia Collier. The allegation was that Collier directed a subordinate to create a new injury claim for an injured worker after an Industrial Commission of Ohio hearing officer had ruled in April 2010 that a recent injury was an aggravation of an allowed, or approved, 2004 claim. Collier also directed a subordinate to file an appeal of the hearing officer's order using the new claim as the basis of the appeal. The new injury claim was manufactured at the direction of Collier, without the knowledge and consent of the injured worker, and for the purpose of Collier gaining an advantage during her appeal of the hearing officer's decision. The injured worker, upon being notified of the new claim, objected to the filing.

On May 9, 2017, the Office of the Ohio Inspector General received a referral from the Ohio Bureau of Workers' Compensation (OBWC) alleging that OBWC Accounts Receivable Manager Michelle Wedemeyer had failed to comply with OBWC policy. OBWC indicated that Wedemeyer was directed to ensure that employer refunds of account credit balances were issued prior to the implementation of a new computer system, PowerSuite, on November 14, 2016. During a review of the employer refund process, the OBWC Internal Audit Division noted that Wedemeyer had split employer credit balance refunds into two separate amounts and issued two separate payments to avoid having to obtain approval from the OBWC chief of Fiscal and Planning.
